The reporter conveyed there were no distractions, just a loss of situational awareness. It was noted by the reporter that a Switchman was on the ground ensuring all switches were in the correct position. The reporter was focusing on the Switchman and did not pay attention to the speedometer. Increased coaching/mentoring for new employees; yard familiarization during different shifts
6352CA4057 The reporter, an Engineer, stated focusing on the freight train distracted the Engineer from the train speed. The Engineer feels that too much was going on in the cab, and the Engineer did not slow down in time. The Engineer stated that Positive Train Control applied a penalty brake application while the train was overspeed. The reporter disclosed the freight train did not ask for a roll by inspection, but the Engineer wanted to be helpful. Share as lessons learned; include in rule of the week or incorporate into an upcoming safety bulletin/message; add as workplace safety topic.

6498CA4151 Employee misunderstood the requirements for a detector stop. Process improvement at the wayside desk when addressing detector failures.

7636CA4279 Mechanical Department failed to inspect outbound cars due to track not solid. Communication between Yardmaster and Mechanical Department to ascertain positive car ID.
7679CA4318 Damaged equipment during flat switching. Switching operations have been reversed. Switching on the steep grade has been eliminated.
7681CA4321 Conductor shoved over an improperly lined derail. Intermediate derails have been removed.
7701CA4332 Yardmasters not train setting trains before calling a crew. Dispatcher concerned about trains carrying dimensional shipments missing high wide restriction on their work orders. Plus conductors and engineers not notifying the dispatcher when carrying dimensional shipments. Yardmaster safety coordinators engaged to brief with terminal teams regarding concerns over high wide paperwork and train setting. Plus high wide awareness video briefing sent to conductors and engineers.
7703CA4335 A train dispatcher notified the police dispatcher that train traffic would be held for an emergency event. However, the train dispatcher released train traffic before notifying the police dispatcher. The train dispatcher did not realize it was their responsibility to notify the police dispatcher. A job safety briefing was created covering emergency events along with a Q&A that covered this particular event. The briefing was emailed and in person briefings were held.
7706CA4337 A Dispatcher issued track bulletin limits to a work train. The work train was authorized to operated past the limits covered in the Track Bulletins. Neither the dispatcher or train crew realized error until the train was out of the limits. A job safety briefing was created covering track bulletin limits. The briefing was emailed and in person briefings were held.
7712CA4342 Dispatcher released a trains joint authority while the train was still operating within joint limits with RWIC. A job safety briefing was created covering releasing joint authority. The job briefing was emailed and in person briefings were held.
7887CA4485 Train dispatcher placed a block causing a proceed signal to drop to Stop. At this location the track is owned by another railroad which causes the control system to respond differently to track blocks. A dispatcher notice was created explaining how track block effect the signal system.
